BLUNDERS by an NHS trust led to two killings by mental health patients, a report declares today.

An NHS review was ordered after Don Lock, 79, was killed last year by paranoid schizophrenic Matthew Daley, 35, in Findon, West Sussex.

Nine other killings by the Sussex Partnership’s mental health patients were probed. Risk checks in seven were “inadequate” or “seriously underestimated”.

Two were “preventable”, including that of Susan Goswell, 63, killed by her husband Roger, 66, after his release from a psychiatric hospital in 2007.

Her son Joe, 46, said: “The trust completely failed my mother.”

The trust issued a “sincere apology”, and has “embedded learning from when things go wrong”.
