A man dressed in black later named as Faisal Hussain, 29, opened fire in the Greektown area of Danforth outside busy restaurants.
Two people were killed – a woman, 18, and 10-year-old girl – with multiple others injured including another young girl aged nine.
The 18-year-old was identified as Reese Fallon, a recent high school graduate who volunteered for Canada's Liberal party – her family said in a statement they were devastated.
Emergency crews were dispatched to near Christina’s restaurant on Danforth Ave shortly before 10.20pm local time on July 22, 2018.
Paramedics treated victims for gunshot wounds, while others were transferred to hospital.
A clip taken from a mobile phone showed a man in a dark hoodie suddenly raise a gun and pull the trigger.
Three gunshots were heard before the person filming lurched to safety.
A witness told the Toronto Sun: "We heard a loud bang and I just looked to my left and I saw a guy in a black hoodie, just standing there, pointing at one of the storefront windows and the glass was just shattered."
Danforth is the main street in Toronto's Greektown - a lively residential area with crowded Greek restaurants and cafes.
It is located between Chester Avenue and Dewhurst Blvd., in east Toronto.
It was named after Asa Danforth who designed Queen Street and Kingston Road.
The area was one of the major settlement areas of Greek immigrants to the Canadian city after the First World War.
The suspect was named by police as 29-year-old Faisal Hussain.
A statement from the family of Hussain said their son had severe mental health challenges that the struggled with psychosis and depression.
They said medications did not help him and the interventions of professionals were unsuccessful.
"While we did our best to seek help for him throughout his life of struggle and pain, we could never imagine that this would be his devastating and destructive end," the Hussain family said.
"Our hearts are in pieces for the victims and for our city as we all come to grips with this terrible tragedy. We will mourn those who were lost for the rest of our lives."
Toronto Councillor Paula Fletcher told CP24 she heard the gunman was emotionally disturbed.
He said: "It's not gang related. It looks like someone who is very disturbed."
It was not immediately clear whether the shooter had been killed by police or by self-inflicted gunshots although Police chief Sanders said the man died after an “exchange of gunfire”.
Toronto Police Chief Mark Saunders said: "We are looking at all possible motives... and not closing any doors."
