The 23-year-old winger started only TWO matches for Liverpool after he was signed by former manager Brendan Rodgers from Sevilla for £6.8m deal in June 2013.
The Spain Under-21 international spent last season on loan at Deportivo La Coruna, where he scored six goals in 29 La Liga matches.
Lazio will pay an additional fee to Liverpool if they qualify for European competition and there is a 30 per cent sell-on clause in the deal.
