JAC Naylor star Rosie Marcel is unrecognisable in unearthed footage of her first appearance on Holby City.
The BBC medical drama airs its last ever episode tonight, with Jac's fate hanging in the balance.
As fans wait to see if the character succumbs to her brain tumour or somehow survives, actress Rosie's first appearance on the show has resurfaced.
The scene saw her burst into a pre-surgery meeting in series eight episode 'Bird on a Wire', which aired in November 2005.
Jac immediately criticises the hospital for its poor signage, revealing to the startled team that she had ended up in the canteen twice.
She then introduces herself properly before the conversation continues.

It was revealed in June last year that Holby City had been cancelled after 23 years, having first aired in January 1999.
The medical drama will conclude tonight after 23 series, and actress Rosie recently opened up about her shock at the show's axing.
The 44-year-old said: "I have loved playing Jac. I thought I would be playing Jac for the rest of my life actually because I loved her. Jac's my friend," she told the Mirror.
"I know she's not a real person but of course, she's me."

She added: "I've taken breaks before. But I've always known that I was going back. When you stop, it's confusing."
Holby City fans will need to brace themselves ahead of tonight's finale episode as they finally discover the fate of Jac.
Desperately clinging on to life, Jac has pushed some of her staff to breaking point by requesting them to perform life-changing surgery on her.
Trusty friend Elliot was ready to operate on her until he was given her latest scan results, showing that the tumour had rapidly grown.
Seemingly accepting her fate, Jac still wanted things to be done her way and convinced Max to let them go through with surgery.

But she requested that Elliot let her die on the operating table if he was unable to extract the tumour.
Eventually, Jac woke up and was distraught to learn that Elliot hadn't been able to remove her tumour.
Holby City airs for the final time tonight at 7.50pm on BBC One.
