This meant that her devastated colleagues were unable to save her when she suffered a deadly clot on her brain.

Jac was declared brain dead minutes after distraught Sacha had begged his fellow medics to administer life-saving drugs.

Fans found themselves in floods of tears at the highly emotional scenes, which mark the end of Holby City's 23-year run on the BBC.

One viewer tweeted: "I’m flat out sobbing."

Chiming in, another added: "Crying, full on crying."

A third posted dozens of broken heart emojis, admitting: "I am absolutely devastated, man."

Holby City has been on screens since 1999 and fans had hoped Jac - played by Rosie Marcel - would make a miraculous recovery in the finale episode.

But just minutes in, she was seen asking her friend Fletch to get a pen and paper and write out an ADRT for her.

She had previously undergone an operation, performed by her colleague Elliot, in an attempt to save her life - but it had not worked.

After being pronounced brain dead after suffering a stroke, her devastated colleagues were seen removing her organs ready for donation.

A list of names and ages were read out, with viewers learning whose lives Jac had saved with her heartbreaking final wishes.

Her voice could then be heard saying: "This is what the NHS means to us. Not a badge on a Cabinet Minister's lapel, not a number down the side of a bus, it's a nurse missing her break to sit with a lonely patient.
