Filming is taking place in the 100-year-old Policlinico Umberto 1 hospital in the centre of Rome, and Cruise himself was on the set earlier this week.
Production vans litter the car park and staff say they are being hindered in doing their job because of the film crew not following proper protocols.
Cruise, 58, has been wearing a mask in line with new regulations as Italy’s Covid cases soar.
Shooting was stopped earlier this year in Venice because of Covid-19 but earlier this month resumed with Cruise and co-star Hayley Atwell flying into Rome for an expected three weeks.
'IT'S MADNESS'
One source at the hospital said:''It's madness we have Covid patients in here and there are dozens of people involved in the set mingling around not wearing masks and not sanitising.
''There is a strict one way system in operation and it's not being followed by the crew. They are using a lift which is also supposed to be out of bounds to keep the virus and any possible contamination at bay.''
The hospital has 1,200 beds with 130 patients being treated for Covid-19, and twelve are in intensive care.
